Insurance/Referrals/Authorizations
Insurance Plans Accepted
We accept Medicare and most PPO insurance plans.
In addition, we are providers for Hill Physicians IPA and Bay
Valley Medical Group.
If it is determined that we do
not have a contract with your insurance company, you may wish to explore
whether your plan’s out-of-network benefits
might allow you to consult with Dr. Gardiner.
Insurance Referral Requests
Please check with your insurance plan to see if you need a referral
request. Since Dr. Gardiner is a specialist, your insurance plan may
require a referral from your primary care physician prior to consulting
with Dr. Gardiner.
Authorizations
Authorization from your
insurance provider is usually needed for tests and surgical procedures.
To obtain authorization, our staff must contact your insurance company
and make a formal request. This process takes time and we ask for your
patience. We can usually request authorization within five working days
following your appointment for routine requests and immediately for
emergent requests. Each insurance company's response time varies. If you
do not hear from our office concerning a request for authorization in
the time frame you expect, please call us to inquire about the status of
the request.
New Conditions
If you are a current
patient with a new condition, please notify our staff.
New conditions often require prior authorization to be treated.
Without authorizations, your appointment may be delayed.
Insurance Co-Payments
Co-payments are due at the time of each office visit.
Payment Options
Please make payments with cash or a personal check.
Our office currently does not accept debit or credit card
payment.
